If you are age 12 or above, please select the option below of how you would like the test to be administered to you. If you choose the online method, a test code as well as instructions will be e-mailed to you upon submission. You will then have 7-days to complete this test. Failure to do so will invalidate the test code and a new one is required.

It's rare for me to give a pain doctor this rating but Dr Horchos deserves it. I have a phobia of doctors after my prior pain doctor managed to have me do something that resulted in my artificial joint being impacted into the bone. I'm still having surgeries to fix what that common wealth Dr did to me. So I went in to this office like a dog that has been beaten too many times.
I was scared. Until Dr Horchos and his staffers actually listened to what I told them, he doesn't cut me off while I explain what's going on, he cares about getting me active again at a safe pace and he never talks down to me and even remembers that I have a special needs sister to care for.that my family doctor of 2 years still forgets about, despite me talking about her every visit.
If other doctors were as professional and caring as Dr Horchos, I wouldn't have the phobia I currently have of doctors. I can't speak to the other doctors here, but Dr Horchos is consistent with his superb care. He also saw my friend's daughter after a lacrosse injury and her mother agreed whole heartedly with everything I said in this post. He is the real deal if you're willing to work with him on a game plan to get back on your feet again.

I had a follow up visit with Dr M Zini P.C. on Wednesday MA y 25th at 3:40. I live in Pawpack Twp. and left my house at 2:55pm more than enough time to make my appointment. On my way there they closed Rte 590 because of accident and I had to take detour. Because of backed up traffic and detour I arrived at 4:00 pm. On my way, about 15 min. away, I called to let them know I would be a little late because of accident.
They told me ok if I'm not more than 15 min. Past my appt.apps. I arrived 20 min. past my appt. and Dr Zini refused to see me. (I use Dr term loosely) I explained to them I live 35 mi. away and couldn't help being late. It didn't matter and they said I would have to reschedule. I'm sorry but no. 1 I was not even aware of the 15 min. late rule and I know Dr's time is important however I would like to think so is my health.
Between distance and gas prices at $4.80 I would like to think Dr. Zini would be at the least a little understanding. Well it wasn't the case and although I didn't get to speak to him I will assume he wasn't concerned enough. I informed secretaries that they should let Dr Chun, who is very professional and understanding, know why he may have lost a patient. I will forward this to Dr Chun and any associated medical agencies. Anthony Kuchemba

It is Tuesday afternoon and I am still trying to get a script ordered last Thursday. Cvs has been requesting info from the office since then and received no response. I have called the office several times and only spoke to receptionists. The last one hung up on me when I asked to speak to a person who could help. How long does it take to return a call?

Dr. Paul Horchos is great Doctor. Yes, he can be frustrating sometimes, just like any other Doctor. But I wouldn't trade him for anything. I had a Line Of Duty Injury, I destroyed my back while working on an Ambulance on March 15, 2005. I went through the standard going to the Company's Doctor and whoever he referred me to with no relief. I have been seeing him since the beginning of May in 2005.
He has never made me feel rushed, he listens to my concerns, he doesn't talk down to me, he is very respectful and he answers my questions when I have them. I would & have recommended for anyone who has been injured and requires a Pain Management Doctor. I have also seen a few of his associates and only had a problem with one of them. Dr. Naftelin (? sp) is a poor doctor in my opinion, because, at least in my case, he violated the first rule of medicine.
It doesn't matter to me weather you practice medicine in the middle of the Street like EMT's and/or Paramedics or completely at the other end of the spectrum, a Doctor especially a specialist. The rule states that you: do no harm I don't know if anyone else has had any problems with him, but I know that I did.

Doctors are fine but the staff does not care about your pain. They take forever to send your medication, and don't do the paperwork right so if your insurance needs prior Auth, you can wait days for them to do their job. And forget if your insurance need a pre-cert for a procedure, it's been 2 months and my insurance is still waiting for thier response.
So because of thier negligence, I'll be sitting at home for days, unable to do anything because of the pain. No procedures, no medication relief, and now going through withdrawal. The worst part is the staff treats you like an addict because you need medication to live a semi normal life. I've never been treated so badly by people who are supposed to help you.
