If this is your first time to our practice or if has just been a while there are a few things that you should bring with you for your visit. Bring your insurance card even if you don't think it covers it. Don't forget your secondary! A list of x-rays, MRI's or other tests you have had done for your problem in the past six months.

For your convenience you may print and complete your intake paperwork at home. Please complete all sections of these forms. Remember to bring the forms, a photo ID and your health insurance card(s) with you to your appointment.

We participate with most of the major health insurance plans that serve central PA. We also accept patients who have auto or work injuries as well as those who pay for their own care. You are responsible to pay your portion of your charges during your appointment. If we do not participate with your insurance plan you should be prepared to pay for your visit before you leave the office, your insurance company will pay your benefits directly to you.We submit all in-network and most out-of-network claims for you.
